Integrity Wellness Center ("we," "our," or "the Center") is a private wellness and clinical testing facility located at 6759 Cedar Ridge Drive, Suite 2, Zephyrhills, FL 33542. We offer counseling, psychoeducational evaluations, clinical testing, and related services to individuals and families in the Tampa Bay Area.
Our services include but are not limited to counseling, psychoeducational evaluations, DNA testing, drug and alcohol testing, nicotine testing, DOT compliance testing, and related professional services. All services are provided by appointment only and are subject to availability.
We reserve the right to modify, suspend, or discontinue any service at any time without prior notice.
Integrity Wellness Center does not provide medical diagnoses, legal advice, or treatment services. All testing results are provided for informational purposes only. For any medical, legal, or diagnostic concerns, you should consult a licensed healthcare professional or qualified legal counsel.
Non-legal DNA tests and discreet testing services are not court-admissible and are provided for personal knowledge only.
All services require a scheduled appointment. We ask that clients provide at least 24 hours notice for any cancellations or rescheduling requests. Late cancellations or no-shows may be subject to a cancellation fee at our discretion.
We reserve the right to refuse service to any individual at our discretion.
Payment is due at the time of service unless other arrangements have been made in advance. We accept cash, credit cards, and other payment methods as indicated at the time of booking. All fees are subject to change without notice.
We are committed to protecting the privacy of our clients. All personal information, test results, and communications are handled with strict confidentiality in accordance with applicable laws. Results are shared only with the client and any parties the client specifically authorizes in writing.
To the fullest extent permitted by law, Integrity Wellness Center shall not be liable for any indirect, incidental, special, or consequential damages arising out of or related to your use of our services or website. Our total liability to you shall not exceed the amount paid for the specific service giving rise to the claim.
We make no warranties, expressed or implied, regarding the accuracy, completeness, or suitability of test results for any particular purpose.
